We believe in being a resource for, reaching out to, advocating for, empowering, and assisting (1) APIs at-risk for HIV infection, (2) those living with HIV/AIDS, and (3) their partners, family members, friends, and service providers to meet their needs and improve access to services.  Delivered by a trained professional staff, our services are confidential and provided free of charge regardless of residency or immigration status.

Medical Case Management

ASIAC’s Medical Case Management services are utilized by individuals who are diagnosed with HIV or AIDS.  The overall goal of the program is to help clients who have HIV/AIDS navigate the health and social services system and ensure that they are receiving culturally and linguistically appropriate medical care.  The ultimate outcome of the program is to create a foundation for medical care that the client can sustain after 1-2 years in medical case management program leading to a longer, healthier life for the client.

Some of the activities include:
•    Connecting a client to an HIV medical specialist and a Primary Care Physician.
•    Ensuring that he/she is taking medications on a regular basis as prescribed by a physician.
•    Coordinating linguistic services for limited English proficient individuals.
•    Assessing the overall medical and psychological well being of the client and connecting him/her to appropriate services, if needed.
•    Assisting clients with applications for disability services, housing services, or food assistance.
•    Connecting the client with immigration services, credit counseling, and substance use counseling, if needed.
•    Providing social support through referrals for caregivers and family members, if needed.

Overall, the Medical Case Manager is the point person for nearly all health and social services the client participates in and typically is the most reliable source of information regarding the health status of a any particular client.

Some other types of information that is recorded in Medical Case Management files are:
•    Viral load/CD4 count
•    Date of last medical appointment
•    All medications the client is taking (HIV, Hep B, Cancer, Psychiatric, etc.)
•    Co-diagnosis information (Hep C, Hep B, HPV, other STIs, etc.)
•    Dental records
•    Any other medical screenings (Mammograms, STI screenings, etc.)

If you live in Philadelphia (or any of the surrounding counties) and have been diagnosed with HIV or AIDS, please call the Philadelphia Department of Public Health AIDS Activities Coordinating Office (AACO) hotline at 1-800-985-2437 (available M-F 8AM-6PM, interpreters available in all languages) and ask to enroll in medical case managment services at ASIAC.

Linguistic Services:

ASIAC can provide a full range of linguistic services free of charge to providers of HIV-related services in the Philadelphia area.  These services include:

  • Document translation
  • Face-to-face interpretation
  • Telephonic interpretation
  • Group interpretation equipment
